    Ceftazidime: pharmacokinetics in young volunteers versus elderly patients and therapeutic efficacy with complicated urinary tract infections

    Thirty-six urological patients (21 male, 15 female) aged 21 to 83 years with complicated and/or hospital-acquired urinary tract infections due to sensitive bacteria were treated with ceftazidime intravenously with a daily dose of 2 g bd over 5 to 17 days. Twenty-seven patients were followed for 1 to 4 weeks after therapy. Cure was observed in 41%, reinfection in 33% and relapse in 26% of the patients. Eradication of the original pathogen occurred in 74%. Five patients showed minor side effects: diarrhoea (2), nausea (1), rash (1), headache (1). No signs of renal, hepatic or haematological toxicity were observed. A pharmacokinetic study was performed in 13 elderly patients aged 63 to 83 years on day 1 of treatment and in 6 volunteers aged 24 to 32 years following administration of 2 g of ceftazidime as short intravenous infusion. The mean serum half life in 12 patients 2.9 h significantly higher than in volunteers (1.75 h). Serum concentrations in patients on day 7 of treatment, however, showed no accumulation when treated with a dosage of 2 g bd

    This paper presents an introduction to the field of three-dimensional medical imaging It presents medical imaging terms and concepts, summarizes the basic operations performed in three-dimensional medical imaging, and describes sample algorithms for accomplishing these operations. The paper contains a synopsis of the architectures and algorithms used in eight machines to render three-dimensional medical images, with particular emphasis paid to their distinctive contributions. It compares the performance of the machines along several dimensions, including image resolution, elapsed time to form an image, imaging algorithms used in the machine, and the degree of parallelism used in the architecture. The paper concludes with general trends for future developments in this field and references on three-dimensional medical imaging

    Pharmacokinetics, in-vitro activity, therapeutic efficacy and clinical safety of aztreonam vs. cefotaxime in the treatment of complicated urinary tract infections

    The minimal inhibitory concentrations (MICs) of aztreonam and cefotaxime were determined against 400 isolates from urological in-patients with complicated and/or hospital acquired urinary tract infections (UTI). Against the Gram-negative rods the activities of both antibiotics were comparable except for higher activity of aztreonam against Pseudomonas aeruginosa. The pharmacokinetic study in nine elderly patients showed a prolonged plasma half life of aztreonam (2.7 h) as compared to younger volunteers (1.6-1.9 h). In a prospective randomized study 39 urological patients with complicated and/or hospital acquired UTI were treated with 1 g aztreonam or cefotaxime iv twice daily for 4 to 15 days. Cure was obtained in 5 out of 18 patients in the aztreonam and 7 out of 20 patients in the cefotaxime group. There were 3 superinfections, 7 relapses and 3 reinfections in the aztreonam group and 1 failure, 1 superinfection, 6 relapses and 5 reinfections in the cefotaxime group. There was no significant difference in therapeutic efficacy between the two antibiotics. Both antibiotics were tolerated well and seem to be equally effective in the treatment of complicated UTI caused by sensitive organisms

    High-performance liquid chromatography analysis of mezlocillin, piperacillin, their degradation products, and of ioxitalamic acid in plasma and urine of healthy volunteers

    In plasma and urine of 10 healthy volunteers after intravenous administration of 4 g mezlocillin and piperacillin, respectively, the parent compounds as well as degradation products were assayed by high-performance liquid chromatography. Ioxitalamic acid, a renal contrast medium, was administered simultaneously, in order to measure the glomerular filtration rate, and to control the collection of 24-h urine. As metabolite of mezlocillin the corresponding penicilloic acid only was found, whereas in the case of piperacillin a further degradation product was observed. Half of the doses given was recovered in the urine as unchanged drugs, and in addition 5-10% as metabolites. No differences were found in the pharmacokinetic behaviour of both antibiotics

    Scalar field and QCD constraints in Nuclear Physics

    Relativistic theories of nuclear matter are discussed in a new pespective. First the chiral character of the scalar nuclear field is introduced in the framework of the linear sigma model. With the assumption that the nucleon mass originates in part from the coupling to the quark condensate it is possible to relate the optical potential for the propagation of the scalar field to the QCD scalar susceptibility of the nucleon, on which indications exist from the lattice evolution of the nucleon mass with the quark mass. Constraining the parameters of the nuclear scalar potential by the lattice expansion parameters a successful description of the nuclear saturation properties can be reached

    PatientenverfĂŒgungen in Deutschland: empirische Evidenz fĂŒr die Jahre 2005 bis 2007

    Informationen zur Verbreitung von PatientenverfĂŒgungen in der Gesamtbevölkerung beruhen in Deutschland meist auf nicht-reprĂ€sentativen kleinen klinischen Stichproben und anekdotischer Evidenz. Aus wissenschaftlicher Sicht sind reprĂ€sentative Untersuchungen bedeutsam, die es neben der Deskription erlauben die Bedingungen zu untersuchen, unter denen Menschen, noch ehe sie persönlich betroffen sind, also im prĂ€klinischen Kontext, bereit sind - oder aber fĂŒr sich selbst ablehnen - eine PatientenverfĂŒgung zu hinterlegen. In vier Erhebungen ergeben sich Ă€hnliche GrĂ¶ĂŸenordnungen von rund 10 % (Bereich: 7-14%) der Erwachsenen, die eine PatientenverfĂŒgung hinterlegt haben. Die aktuellste Erhebung ist eine Sondererhebung der LĂ€ngsschnittstudie "Sozio-oekonomisches Panel" (SOEP) bei 1000 Erwachsenen im Jahr 2007. Danach hat nur jeder zehnte Erwachsene eine PatientenverfĂŒgung erstellt. Kompatibel damit ist der Anteil von 7 %, der in dieser Erhebung fĂŒr nahe stehende Verstorbene angegeben wird. In einer SOEP Sondererhebung im Sommer 2006 gaben etwa 11 % von 400 Befragten an, eine PatientenverfĂŒgung erstellt zu haben. In einer Umfrage der Infratest-Finanzforschung im Auftrag der Deutschen Hospizstiftung gaben dies Ende 2005 14 % an. Wir erlĂ€utern, warum dieser Wert vermutlich eine ÜberschĂ€tzung darstellt. Weitere Analysen zeigen die sozialen, biografischen und bildungsbezogenen Bedingungen, die zwischen den Menschen differenzieren, die eine PatientenverfĂŒgung erstellt haben oder aber fĂŒr sich explizit ablehnen, eine PatientenverfĂŒgung zu erstellen. Die Befunde zeigen, dass neben dem Alter und schlechterem Gesundheitsstatus vor allem die persönliche Erfahrungen mit dem Tod von Angehörige begĂŒnstigt, dass eine PatientenverfĂŒgungen erstelltund hinterlegt wurde.

    Mixing and condensation in a wet granular medium

    We have studied the effect of small amounts of added liquid on the dynamic behavior of a granular system consisting of a mixture of glass beads of two different sizes. Segregation of the large beads to the top of the sample is found to depend in a nontrivial way on the liquid content. A transition to viscoplastic behavior occurs at a critical liquid content, which depends upon the bead size. We show that this transition can be interpreted as a condensation due to the hysteretic liquid bridge forces connecting the beads, and provide the corresponding phase diagram.Comment: submitted to PR

    Roughness Induced Rotational Slowdown Near the Colloidal Glass Transition

    Roughening the surface of spherical colloids can drastically change their translational and rotational dynamics in dense suspensions. Using 3D confocal microscopy, we show that roughness not only lowers the concentration of the translational colloidal glass transition, but also generates a broad concentration range in which the rotational Brownian motion changes signature from high-amplitude diffusive to low-amplitude rattling. This hitherto not reported second glass transition for rough spherical colloids emerges when the particle intersurface distance becomes comparable to the roughness length scale.     Abiotic methane synthesis and serpentinization in olivine-hosted fluid inclusions

The definitive version was published in Proceedings of the National Academy of Sciences.of the United States of America 116(36), (2019): 17666-17672. doi:10.1073/pnas.1907871116.The conditions of methane (CH4) formation in olivine-hosted secondary fluid inclusions and their prevalence in peridotite and gabbroic rocks from a wide range of geological settings were assessed using confocal Raman spectroscopy, optical and scanning electron microscopy, electron microprobe analysis, and thermodynamic modeling. Detailed examination of 160 samples from ultraslow- to fast-spreading midocean ridges, subduction zones, and ophiolites revealed that hydrogen (H2) and CH4 formation linked to serpentinization within olivine-hosted secondary fluid inclusions is a widespread process. Fluid inclusion contents are dominated by serpentine, brucite, and magnetite, as well as CH4(g) and H2(g) in varying proportions, consistent with serpentinization under strongly reducing, closed-system conditions. Thermodynamic constraints indicate that aqueous fluids entering the upper mantle or lower oceanic crust are trapped in olivine as secondary fluid inclusions at temperatures higher than ∌400 °C. When temperatures decrease below ∌340 °C, serpentinization of olivine lining the walls of the fluid inclusions leads to a near-quantitative consumption of trapped liquid H2O. The generation of molecular H2 through precipitation of Fe(III)-rich daughter minerals results in conditions that are conducive to the reduction of inorganic carbon and the formation of CH4. Once formed, CH4(g) and H2(g) can be stored over geological timescales until extracted by dissolution or fracturing of the olivine host. Fluid inclusions represent a widespread and significant source of abiotic CH4 and H2 in submarine and subaerial vent systems on Earth, and possibly elsewhere in the solar system.We are indebted to J.     Nanometer-Resolved Collective Micromeniscus Oscillations through Optical Diffraction

    We study the dynamics of periodic arrays of micrometer-sized liquid-gas menisci formed at superhydrophobic surfaces immersed into water. By measuring the intensity of optical diffraction peaks in real time we are able to resolve nanometer scale oscillations of the menisci with sub-microsecond time resolution. Upon driving the system with an ultrasound field at variable frequency we observe a pronounced resonance at a few hundred kHz, depending on the exact geometry. Modeling the system using the unsteady Stokes equation, we find that this low resonance frequency is caused by a collective mode of the acoustically coupled oscillating menisci.Comment: 4 pages, 5 figure
